APPOINTMENT
We require a face-to-face meeting with our clients prior to representation
so that we can discuss and understand their needs and they understand our
abilities to meet those needs. The request for an appointment does not mean
that we are your attorney.
There are 2 ways to request an appointment
and free initial interview please select the one that is best for you.
*1. Schedule an appointment right now:

b. Complete our online short questionnaire

To provide a better meeting, please bring with you:
(1) Copies of your paystubs
for the current month and for the six (6) preceding months plus this month;
(2) A listing of all of your monthly expenses;
(3) A list of the
names and addresses of all creditors (You may
want to go
www.annualcreditreport.com
to obtain your free credit report.)
(4) All collection, lawsuit and foreclosure
information that you have; and
(5) Any other information that you think may
be useful to us.
*2. Complete homework using a downloadable PFD form:
a. Complete

b. Email questionnaire to

or bring it with you on your first visit.
c. Complete Homework for a $100.00 credit toward your total fee.

will go much smoother by giving us vital information in the initial meeting.
At present, the homework pages are under construction. However,
please feel free to try them and email your comments to
web@lusky.com.
Homework are the documents that you will need to complete in order for
us to process your case. While, in an emergency, a case can be filed without
the completion of these documents, we prefer to have all of the documentation
in final form prior to filing. This is because there are relatively short
deadlines for filing the documents in final form. For example, the deadline
for filing the matrix (list of names and addresses of creditors in a form
that can be scanned) is 48 hours from filing the case. If the deadlines
are not met, the case is subject to being dismissed without further notice.
Simply put, having all documents completed prior to filing makes less work
for us and less cost to you.
If you complete your homework prior
to meeting with us, you can receive up to $100 credit against your total
fee.

Get Homework:
You may fill out the forms securely on line. Simply click on the following
link. When asked for attorney ID, type in "lusky." You do not need to complete
the homework in one sitting. You may save your work and come back to it
later.
This is a third-party site and does NOT give you the information
that we would normally give you about completing your schedules.
When you complete the homework, let us know by phone and set an appointment
to review it. Make sure to tell us that you have filled out the homework
online so that we will be prepared for the interview.
